The insurance covers life protection and a full range of private medical care in the LUX MED network - from teleconsultation to in-patient visits, tests and treatments to rehabilitation.

  • 24/7 teleconsultation - internist, family doctor, pediatrician: up to 3-4 hours from notification, unlimited
  • Specialist teleconsultations - 17 (basic) or 24 (extended) specialists, up to 3 working days
  • Inpatient visits to specialists - 16 (basic) or 33 (extended) specialists, up to 5 working days, surcharge 40 PLN
  • Laboratory tests - Hematology, coagulology, lipidogram, glucose, hormones, tumor markers and more

Specialists available on an inpatient basis: allergist, surgeon, dermatologist, diabetologist, endocrinologist, gastroenterologist, gynecologist, cardiologist, laryngologist, neurologist, ophthalmologist, oncologist, orthopedist, pulmonologist, urologist, venereologist - and in the extended option additionally andrologist, hematologist, nephrologist, neurosurgeon, psychiatrist, rheumatologist, rehabilitation specialist, among others.

LUX MED network - the largest private care in Poland

The Medical Plan is implemented in the network of LUX MED facilities - the largest private medical center in Poland with over 20 years of experience. Access to diagnostics, specialized treatment, rehabilitation and continuous care. Non-referral visits, booking online or by phone. Premium independent of age - You pay the same amount being 25 or 65 years old.

Contribution and paperwork

The amount of the premium depends on the selected variant (basic/extended) and form (individual/partnership/family), but does not depend on age. The premium can be paid monthly, quarterly, semi-annually or annually. The contract is annual with automatic renewal.

Insurance can be taken out by a person of age 18-70 years residing in Poland. Co-insured (children under 25, partner) must also live in Poland. After termination, another contract can be concluded after 6 months at the earliest.

How to buy UNIQA Medical Plan insurance

  1. Choose a variant and form - Basic or extended; individual, partnership or family.
  2. Fill out an application - Personal data of the insured and any co-insured.
  3. Pay your contribution - Monthly, quarterly, semiannually or annually.
  4. Create an account on the LUX MED platform - address given in the policy, PESEL needed.
  5. Enjoy the benefits - Book appointments online, by phone or through the platform.

Exclusions and limitations

  • Pregnancy and postpartum - treatment related to pregnancy and childbirth is not covered
  • Pre-contract chronic diseases - treatment of diseases diagnosed before coverage
  • Aesthetic medicine - cosmetic and aesthetic treatments
  • Dental treatment - Dentistry outside the scope of coverage under the policy
  • Non-compliance with doctor's recommendations - Failure to implement the recommendations of the attending physician
  • Out-of-network facilities - benefits in establishments that do not cooperate with LUX MED
